Naturally, you will need a Square Enix account in order to access the service. So if you do not already have one, you will need to make one to log in and play Final Fantasy XIV. Additionally, you do not need PlayStation Plus to play Final Fantasy XIV. So the only thing you will be required to have after your 30-day free trial, is a subscription to the game.
